Dietary supplement — not a treatment for erectile dysfunction

Is EreFil safe?

Start with the appointment, not the capsule

An erection depends on healthy blood vessels, and so does your heart. Both problems often begin with the same thing — damage to the lining of the blood vessels — and the arteries supplying the penis are narrower than those supplying the heart, so they show trouble first.

Mayo Clinic describes erectile dysfunction as an early warning sign of current or future heart problems, and notes it can be the first significant indication that arteries are narrowing. In Mayo research, men aged 40 to 49 with erectile difficulty were around twice as likely to go on to develop heart disease. A 2024 consensus panel urged clinicians to treat it as a cardiovascular risk marker rather than a lifestyle complaint.

A doctor can check blood pressure, cholesterol, blood sugar and testosterone, and can catch something treatable early. Surveys suggest nearly one in five men over 55 would not raise this with a professional. That reluctance is the real risk in this subject, not the supplement.

See a doctor promptly if

The change was sudden or is worsening; there is chest discomfort, breathlessness on exertion or unusual fatigue; or there are urinary changes such as a weak stream, difficulty starting, or repeated waking at night. Those last ones point at the prostate and need proper assessment.

The adulteration problem in this category

The FDA has recalled horny goat weed products found to contain undeclared sildenafil — the prescription drug in Viagra — sold as a herbal supplement. For anyone taking nitrates for angina, that combination can cause a dangerous drop in blood pressure. Buy only through official channels, and treat any product promising drug-like results as a warning sign.

Do not take it without medical advice if you use

Nitrates for angina. Sildenafil, tadalafil or similar ED medication — overlapping PDE5 mechanism. Blood pressure medication. Blood thinners or antiplatelet drugs — several ingredients here affect bleeding. Finasteride or dutasteride — saw palmetto acts on the same pathway. Diuretics or lithium — nettle. Anything processed by CYP3A4, which epimedium is documented to affect.

Before a PSA test

Tell your doctor you are taking a saw palmetto product. Evidence on whether it suppresses PSA readings is mixed, but anyone interpreting a prostate screening result should know what you are taking. Do not let a supplement quietly complicate a cancer screening.

Who should not take it

Anyone under 18. Women, including during pregnancy and breastfeeding. Anyone with a hormone-sensitive condition, heart disease, or liver or kidney disease, without their doctor's agreement.

Possible side effects

Most people tolerate botanical formulas of this kind. A minority notice mild digestive upset, headache or restlessness early on. Stop and seek advice for a rash, unusual bruising or bleeding, or chest discomfort. An erection lasting more than four hours is a medical emergency — go to hospital — and is a genuine risk if a product has been adulterated.

What this product is not
